Durable Power of Attorney for Financial Matters

The Durable Power of Attorney for Finances appoints an individual to manage the finances and property of the person being cared for when they are no longer able to do so themselves. The form can be completed by the caregiver and then needs to be signed with at least two witnesses who know the person who is being cared for or by a notary public.

Where to Keep This Form

Copies of the Durable Powers of Attorney should be given to the individual’s agent(s). Copies should also be kept in an envelope on the refrigerator or another location where the copies are easily accessible in case of an emergency.
